Hello. I need help desperately. LOL

I've been using the spring ldap 1.1.2 in my application that we deployed throughout several installations around the world. There is, however, only one place where my application returns the following error message upon user login:

LDAP: error code 48 - Bind failed: Username / password binds not allowed

Before getting into other details, is this something anybody has seen before? If so, what is causing it?
