Form Data not saved in DB

I am a new to Spring tool suit and I am building a small application which have user form and I want to save that form data in MySQL using persistence and JPA.

But I am stucked at some exceptions like http request is not mapped.

Please suggest me some code.

Before you post, do a google search for blogs and tutorials teaching you how to do this. If that doesn't work, you can post the question. When you do post, make sure to be more specific. Show exactly what you tried. Post some code and describe what doesn't work.
